To lodge an objection

You can lodge an objection in several ways:

Log onto Mijn CAK

Please note

Did the Wmo care start after 1 January 2020? Then you cannot yet use Mijn CAK to object. Please use the form.

Print objection form

Print the objection form (20-2-2022, pdf, 946 kB, in Dutch only) and fill it in. Describe why you disagree with a particular decision. Also put your signature on the form.

Writing a letter

Would you rather write us a letter? Send us a notice of objection and include the following information:

• your name and address
• the date on which you send us the notice of objection
• a description of the decision to which you are objecting (state date and reference)
• the reason why you disagree with the decision
• your signature
• your phone number

Include a copy of the decision you are objecting to.

Are you unsure whether you should object to a certain decision? Please contact us by phone. We will then determine together whether an objection is necessary or whether we can help you more quickly in another way. 

Do you disagree with a decision? Then call us for free first. We will then determine together whether an objection is necessary. We might be able to help you faster in another way.

Do you still want to object? You have 6 weeks to do so. There are 2 ways in which you can lodge an objection:

• by filling in a form and sending it to us or
• by writing a letter.

You cannot lodge an objection by telephone.

Print objection form

Print the objection form (13-09-2017, pdf, 64 kB, Dutch only) and fill it in.

Note: do not forget to enter your telephone number and signature. This way we can easily reach you if we have any questions.

Please enclose the following documents with your form:

• a copy of the decision to which you are objecting
• copies of supporting documents
• if someone else objects on your behalf: an authorization (13-09-2017, pdf, 45 kB, Dutch only)
