Are you planning to emigrate to Belgium or are you already living there? And will you be covered by health insurance through the CAK? This page provides information about your health insurance in Belgium.
Health insurer/agency
After registering with the CAK, you will receive an S1 form. Submit this form to an health insurance fund of your choice:
- A health insurance fund affiliated with one of the five national health insurance associations; or
- the Sickness and Disability Insurance Fund (HZIV).
Reimbursements
In Belgium
- Basic package: includes GP, hospital, and medication.
- If you find the reimbursements too low, you can take out supplementary insurance (hospitalisatieverzekering) with your health insurance fund.
- If you live in Flanders, you are also entitled to benefits such as home care, elderly care, residential care, long-term rehabilitation, or mental health care from the Vlaamse Sociale Bescherming. This scheme is free for you. If you wish to use it, please inform your health insurance fund.

In the Netherlands
- Basic package: includes GP, hospital, and medication; see the Zilveren Kruis website.
- No personal contribution or deductible.
- On presentation of the EHIC from the CAK.
In another treaty country
- Medically necessary care (care that cannot wait until you return home).
- You must present your EHIC from the CAK.
- Tip: always take out travel or supplementary insurance as well.
What do you pay?
- A healthcare contribution to the CAK . Calculate your treaty contribution with the CAK - Healthcare Contribution Calculator (in Dutch). We deduct this contribution monthly from your pension(s) or benefits.
- A monthly membership fee of approximately €10.00 to your Belgian health insurance fund. HZIV is free of charge.
- A personal contribution for medical expenses in Belgium: remgeld en liggeld (never more than half, with a maximum amount per year).
- Supplementary insurance (hospitalization insurance) is at your own expense.
Tip
- Apply for healthcare benefits from the Dutch Tax and Customs Administration.
- Are you a taxpayer in your country of residence? In some countries, you can deduct your healthcare contribution. Contact the tax authorities in your country of residence to discuss the options.

Your family members (partner, children) without their own income who live in Belgium are eligible for co-insurance. However, the Belgian health insurance fund must approve this.
