Are you planning to emigrate to Spain or are you already living there? And will you be covered by health insurance through the CAK? This page provides information about your health insurance in Spain.
Health insurer/agency
After registering with the CAK, you will receive an S1 form. Submit this form to the local office of the Spanish health insurance fund, the Instituto Nacional de Seguridad Social.
Pleas note: Please inquire at the office about the documents you need for registration. This varies by region, but in any case:
- A Certificado UE (also called residencia or green card), proof that you are registered as an EU citizen in Spain. You can apply for this card at a local Policía Nacional office.
- A NIE number, similar to a Dutch citizen service number (BSN). You can apply for an NIE at a local Policía Nacional office or through the Spanish consulate in Amsterdam.
Tip
Schedule appointments with these agencies ahead of time. Wait times can be long.
Reimbursements
In Spain
- Public medical care: including GP, hospital and medication through the Spanish health insurance fund (Seguridad Social).
- Check with your local Seguridad Social office to find out exactly what care is covered.
- Do you find the reimbursements too low or the package too limited? Contact Seguridad Social to inquire about supplementary insurance options.
In the Netherlands
- Basic package: includes GP, hospital, and medication; see the Zilveren Kruis website.
- No personal contribution or deductible.
- On presentation of the EHIC from the CAK.
In another treaty country
- Medically necessary care (care that cannot wait until you return home).
- You must present your EHIC from the CAK.
- Tip: always take out travel or supplementary insurance as well.
What do you pay?
- A healthcare contribution to the CAK . Calculate your treaty contribution with the CAK - Healthcare Contribution Calculator (in Dutch). We deduct this contribution monthly from your pension(s) or benefits.
- A personal contribution for certain medical care en medication in Spain.
- Supplementary insurance (hospitalization insurance) is at your own expense.
Tips
- Apply for healthcare benefits from the Dutch Tax and Customs Administration.
- Are you a taxpayer in your country of residence? In some countries, you can deduct your healthcare contribution. Contact the tax authorities in your country of residence to discuss the options.

Details
Your family members (partner, children) without their own income who live in Belgium are eligible for co-insurance. However, the Belgian health insurance fund must approve this.
