The steps you need to take depend on your family's country of residence.
My family lives in a EU/EER-country, Switzerland or the United Kingdom
- Apply for a document S1 for your family members. You can download the application form here: Application for a document S1 family member (pdf, 516 kB) (01-06-2022).
- Fill out a separate form for each family member and upload the form through our contact form (select scheme Health insurance outside the Netherlands) or send by post to: CAK, Regeling Buitenland, Antwoordnummer 91041, 2509 Den Haag.
- When approved your family members will receive an S1. With this document they need to register with the local health insurer in their country of residence.
My family lives in Bosnia and Herzegovina, Cape Verde, Morocco, Montenegro, North Macedonia, Serbia, Tunisia or Turkey
- Apply for a document 109 for your family members. You can download the application form here: Application for a document 109 family member (pdf, 504 kB) (01-05-2023).
- Fill out 1 application form and upload the form through our contact form (select scheme Health insurance outside the Netherlands) or send by post to: CAK, Regeling Buitenland, Antwoordnummer 91041, 2509 Den Haag.
- When approved your family members will receive 1 form 109. With this document they need to register with the local health insurer in their country of residence. Please note: this form only states the name of your eldest family member. The local health insurer checks which family members can be co-insured and will write their names on the back of the form.
What happens next?
The local health insurance fund checks if your family member(s) can be co-insured and will notify us. You will receive a letter from us stating which family members are co-insured. Your co-insured family members will receive a health insurance card from the local health insurer or a proof of registration. With that they will receive the same healthcare as other residents of their country of residence.
Tip
Does the local health insurer decide that your family member cannot be co-insured via the CAK? Or does your family live in another country then the treaty countries? In that case arrange health insurance for them in their country of residence yourself.
