You pay a healthcare contribution for basic healthcare in the country where you live. And if you live in an EU/EEA Member State, Switzerland or the United Kingdom, you are also insured for basic healthcare in the Netherlands and medically necessary care in a number of European countries.

If you're insured in the Netherlands and your family have no or little income of their own,  the Netherlands is also responsible for the healthcare costs of your family. They are then insured through the CAK.

If you have a pension or benefit from the Netherlands and live in a treaty country, then you have to arrange and pay for your health insurance through the CAK. This way, you are always insured for medical care in your country of residence.
